This page is a template. Copy the questions into Google Forms / Typeform / SurveyMonkey and send from there.
- Copy the question text on this page into Google Forms / Typeform / SurveyMonkey to distribute.
- This is an executive audience โ keep required questions to 2โ3. This template marks Q1 / Q2 / Q4 / Q5 as required.
- All free-text questions (Q7 / Q8) are optional.
- Distribution timing: embed the URL in the T+1 follow-up email (see "T+1" block in facilitator-emails.html).
- Paste ยง3 Copy-paste plain text into Forms to port the whole survey in ~5 minutes.
Privacy policy: Responses are collected without name, role, or email. If any free-text response in Q6 (barrier selection) or Q7 (free text) contains identifying information, the raw submissions are deleted after the tally review (within ~2 weeks).

Google Forms porting tips: Forms' "Import questions" feature only works from another Form. There is no automatic import from plain text, so add each question manually (~10 minutes for one person). Don't forget to toggle Q1 / Q2 / Q4 / Q5 to "required".
ยง4 Connection to ROI measurement
This survey alone only measures day-of satisfaction. ROI only becomes visible when you connect three measurement points โ short / medium / long.

ROI math (quick estimate): (Q5 hours/week) ร 52 weeks ร executives = annual hours saved. The facilitator-script CLOSING line "2 hours/week per executive = 104 hours/year saved" is validated against Q5. If the T+30 actuals come in under 50% of the Q5 estimate, the rollout process has a problem โ treat that as a red flag.
ยง5 Response handling
- Aggregation: Use Google Forms' built-in aggregation (spreadsheet-linked). No separate BI tool needed.
- Review cadence: 30-minute review in week X+1 with Corporate Strategy + facilitator.
- Where to apply learnings: Feed changes into the next-round facilitator script and Driver View.
- Deletion rule: Delete the raw submissions of identifiable free-text (Q7 / Q8) after tally review (~2 weeks). Keep only the quantitative aggregates (Q1โQ6) in the spreadsheet.
- Internal sharing: Raw results are restricted to Corporate Strategy + facilitator. Only a quantitative summary goes to the wider executive group.
Next-round improvement loop: Q1 average below 4.0 โ restructure sections / Q3 top 3 โ add +5 min to those sections next time / Q6 top 2 โ pre-empt those barriers in next-round handouts. Run it as a template.
